In this article, you can read about the basic setup of feedback forms, which is the second step in starting the work with feedback - e.g. for a performance development review.

Create/edit a feedback form
1. Go to "Settings" in the main menu and click "Feedback"
2. Tap on the type of feedback created to create feedback
3. Press "New feedback" and give the feedback a title
Press Save to continue.
4. Tap "Add Step" and specify title
5. Press "add element"
6. Select the element type you want to insert
Score: this element allows for a rating/evaluation on a scale from x-x (1-10 of your choice). Here, you can also choose the 'Don't know' option. (Please note that forms used without that option to start with will not allow choosing that option, so data will not be corrupted).
Text answer: this element allows for an answer in the text. This can be in one or more lines
Options: this is an optional element. It can be with one or more answer options
Date: this element is useful if you want a specific date as an answer
File: this element allows uploading of a given file
Number: this element allows you to specify a "number" as the answer
NOTE! Common to all of the above is that elements only need to be added the "first time". If an element is already on another feedback form, you must find the elements under "Existing elements"
7. Fill in the content about the new element (what questions and opportunities do you want to use?)
In the example above, a "score element" is selected, indicated by "5 answer options"
8. Fill in any internal settings for the element
It is advantageous to give the element an internal category or description.
"Help text" is very useful in relation to a short info message attached to the rating/answer
"Advanced" allows you to activate whether the element should be available on other feedback forms and whether the answer should be available for reporting/statistics themselves, this should be deleted in the future.
9. Return to the basic settings and press "Settings"
10. Select the access type for the roles. (See own feedback, all or custom)
11. Fill in the navigation settings
12. Fill in "Visibility"
Here, you will have two settings that you can enable:
Show responses on the Feedback tab: If enabled, it will show the feedback form with responses on the main Feedback tab. If disabled, filled feedback forms/responses on the main feedback tab till not be visible.
It is possible for the user to decide whether this setting should only be enabled for some roles by deselecting specific roles in the "All roles" dropdown.
Show only when at least one has responded: If enabled, the feedback form/responses are only visible on the main Feedback tab if at least one person has filled out the form.
13. Fill in "Display parameters" if necessary (hide elements until certain answers are given)
Finish by pressing "Save" in the upper right corner
